According to a recent post by Dan Pinal, it happens
when you quit a guild, or kill a hear (or pauper?). I
think they might've had something to do with the
Troll/Goblin quest, but that may be a superstition I
had... I had tons of 'em, like 'praying' for a healer
when you got the loading screen would somehow actually
summon one.

I loved the fact that the Assasins say "greetings from
a friend..." and the fact that I don't think I've ever
been killed by one - I was too high level by the time
I had made any enemies... And they never managed to do
it in their first hit.
